Join the Sonoran UCEDD for a free webinar "A Closer Look at Supported Decision Making" on Dec. 18 at 9:00 a.m. MST

Guest speaker Valerie Mata from the UArizona Sonoran Center for Excellence in Disabilities will share how Supported Decision Making can help you live independently and manage your health and wellness.

Guest Speaker:
Valerie Mata is the Medical Home Coordinator at the Sonoran UCEDD. She provides case management for adults with developmental and intellectual disabilities at Banner Family and Community Medicine South Clinic. These services are provided in a primary care clinic that is patient centered and sensitive to health care needs of patients with disabilities. Her role is to ensure our patients are linked to both medical and non-medical community resources. Prior to joining the UCEDD, Valerie spent 18 years as a K-12 special education teacher in the public education system.
