Vaccination Event!

Must be 5 years old or over. Minors must have an Authorized Health Representative for consent. Vaccinations will be administered in a walk-in clinic setting at the Ability360 Sports & Fitness Center gymnasium. ADA reasonable accommodations and American Sign Language (ASL) will be provided upon request in the sign-up system. An appointment is required.

Transportation

The AHCCCS Health Plans are providing transportation to vaccination.  Members would need to call the phone number on the back of the their AHCCCS insurance cards or go to this link for their Health Plans contact information https://www.azahcccs.gov/AHCCCS/Downloads/COVID19/AHCCCSHealthPlanNEMTcontact.pdf

Area Agency on Aging will provide Free transportation to vaccine shots for individuals 60 years or older in Maricopa County.  Contact Derek Oetken  602-241-5562.
