Future Planning: A Roadmap of Our Futures, Our Wishes is a workbook about how to facilitate future planning.
This involves gathering information, exploring choices, sharing dreams, documenting wishes, reaching goals, and creating a written plan. It includes plans for the person with a disability and for caregivers as their roles change over time.
This roadmap is designed as a tool to be used by individuals with intellectual and developmental disabilities, family members, direct support professionals, and advocacy groups to guide the planning process.
