Future Planning Workbooks
Future Planning: A Roadmap of Our Futures, Our Wishes is a printable workbook helping families plan for the future with a loved one with a disability. It offers peace of mind to family members and the individual and provides guidance on how to address difficult topics, including expectations, finances, caregiver transition and the wishes of the individual with a disability. This project arose from an ADDPC grant to the University of Arizona-based Sonoran University Center for Excellence in Disabilities. Click here to read more and download the workbook.

Preventing Abuse: Questions Families and Individuals Can Ask
As part of efforts to prevent and raise awareness about sexual abuse of people with disabilities in Arizona in the aftermath of the Hacienda HealthCare investigation underway in early 2019, the ADDPC created a set of resources for families. This downloadable infographic provides a starting point for families to ask service providers and residential facilities that care for a loved one with a developmental disabilities.

Arizona Employment Provider Directory
The Arizona Employment Provider Directory lists key employment services offered by employment providers in the state. While not a comprehensive list of all services, the info is a result of employment providers who responded to an ADDPC survey. Some of the services listed may not be funded by the Arizona Division of Developmental Disabilities or Vocational Rehabilition. Please confirm with the provider which services are funded.
